Adding a Sign-Up.to data capture form to your Facebook page
The Sign-Up.to Form Manager Facebook App gives you a simple interface for adding your Sign-Up.to data capture forms to your business’ Facebook page. This is great for getting your Facebook fans to sign up for your email newsletter or promotions.
Please note you must have created a data capture form in Collect and integrated your Facebook page with your Sign-Up.to account before you can add a form to your Facebook page.
You can also embed a data capture form on your website.
How to use the Form Manager App on your Facebook page
- Head over to the App’s page on Facebook to access the App.
- From the ‘fan page’ drop down, select the page you would like to add your data capture form to.

- Select your Sign-Up.to form from the list. You will see a preview of the selected form in the area below.
- You then have the option to restict access to your form to only those people who ‘Like’ your business page. Choose ‘enable’ to do this.

- If you enable this function, you can enter a message or some HTML code in the box below to encourage people to ‘Like’ your page.
- Click ‘continue’.
- On the next screen, choose which page you want to add the application to and click the ‘add Sign-Up.to Form Manager’ button to complete the process and be returned to your Facebook business page.

That’s it! You should now see ‘Sign-Up.to form’ tab under the cover image on your page.
How to rename and change the image on the form tab
- Click on ‘edit page’ in the top right of your Facebook business page and select ‘manage permissions’ from the drop down menu.

- Navigate to the ‘apps’ tab on the left hand menu.

- Under the Sign-Up.to Form Manager App click ‘edit settings’.

- In the settings window enter the name you’d like for your tab and click ‘save’ and then ‘OK’.
- To change the image, simply click the ‘change’ link next to the custom tab image option.
How to edit the App settings
- Click on ‘edit page’ in the top right of your Facebook business page and select ‘manage permissions’ from the drop down menu.

- Navigate to the ‘apps’ tab on the left hand menu.

- Under the Sign-Up.to Form Manager App click ‘go to application’.

- Here you can change which form shows on which pages, as well as amend the settings for restricting access to your form to only fans.
How to remove a form from a page
- Click on ‘edit page’ in the top right corner of your Facebook business page and select ‘manage permissions’ from the drop down menu.

- Navigate to the ‘apps’ tab on the left hand menu.

- Find ‘Sign-Up.to Form Manager App’ in the list and click the small cross on the right.

- Click ‘remove’ to confirm.

Please note that removing a form is page specific and will not affect any other Sign-Up.to data capture forms you may have on other business pages.
